: Applied Economics and Health Research received a grant from the Danish Diabetes Association to conduct the analysis. The Danish Diabetes Association has not had any access to data, analyses or conclusions from this paper. AS reports receiving lecture fees for providing continuing medical education to GPs. SJG is a National Institute of Health Research (NIHR) Senior Investigator and member of the NIHR School for Primary Care Research.
