: The CONVERT trial is registered on ClinicalTrials.gov (identifier: NCT00433563). All participants provided written informed consent, and the study adhered to the ethical principles outlined in the Declaration of Helsinki and Good Clinical Practice guidelines. Ethical approval for the trial was granted by the National Research Ethics Service Committee Northwest–Greater Manchester Central in the United Kingdom on December 21, 2007 (REC reference 07/H1008/229).
