: This study was performed in line with the principals of the Declaration of Helsinki. Ethical approval was granted by the Ethics Committee at Lund University, Sweden (EPN 2014/717), waiving the requirement for further informed consent.
: Consent to participate, with possibility to opt out at any timepoint, in studies involving the archived tissue was given at the time of tissue submission and referral to the pathology department. As this study had a retrospective design, the ethics approval waived the requirement for written informed consent.
