: Lina M. Serna-Higuita and Peter Martus kindly provided statistical advice for this manuscript.
: Written informed consent was obtained from all subjects (patients) in this study.
: Institutional Review Board approval was obtained from all sites. This study was approved by the ethics committee at Charité–Universitätsmedizin Berlin as the coordinating centre, by the German Federal Office for Radiation Protection, and by local or national ethics committees of all sites.
: The patients in the current submission were recruited for the EU-funded pragmatic, randomised, multicentre DISCHARGE trial (NCT02400229, Eur Radiol. 2017 Jul;27(7):2957–2968) to assess the comparative effectiveness of CT and ICA in patients with stable chest pain clinically referred for ICA with an intermediate clinical pre-test probability of obstructive coronary artery disease. We randomly assigned 3667 patients at 26 clinical centres in 16 European countries to a CT-first or an ICA-first strategy. The main results were published in the NEJM in April 2022 (NEJM 2022 386(17): 1591–1602).A pre-specified subgroup analysis not included in the main DISCHARGE trial outcome paper published in the NEJM was to evaluate the impact of smoking status on the effectiveness of CT vs ICA in symptomatic patients referred for ICA. Herein, we submit the results of this pre-specified subgroup analysis.
