: The study was approved by the local regional ethics committee “REK Vest - reference number 2017/930”. All the procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and/or national research committee and with the 1964 Helsinki Declaration and its later amendments or comparable ethical standards.
: Since the study was retrospective, and many of the participants were already deceased—a general exemption from the rule of informed consent was given by the regional ethics committee. A letter to all living participants was sent where information about the study was given with a possibility to deny participation. No participants denied.
