: The study protocol was approved by a Scientific Committee, consisting of external experts, the cooperating sickness funds, the research sponsor, Cytel and IPAM. As the study analyzed retrospective anonymized claims data, the use of such data is not subject to ethics committee approval and no informed consent was required according to German laws (§ 75 SGB X, Tenth Social Code Book). The work on the dataset conformed to all social security data protection requirements. The study followed all the dictates of the Declaration of Helsinki.
