: Prior to the commencement of the study, ethical approval was obtained from the School of Psychology Ethics Research Committee at the University of Leeds, UK (Reference: 10-0105, Date: 27/12/2010) and was conducted in accordance with the Helsinki Declaration of 1964 and its later amendments.
: Informed consent was obtained from parents using passive consent (opt-out), and each child gave his/her own verbal assent to participate in the study at screening (opt in).
