: Ethical approvals for baseline survey were obtained from the Oxford Tropical Research Ethics Committee (OXTREC) at the University of Oxford and the Chinese Center for Disease Control and Prevention (CDC) Ethical Review Committee. Ethical approvals for the 2013–14 resurvey were obtained from OXTREC and the Chinese Academy of Medical Sciences/Peking Union Medical College Ethical Review Committee.
