All study methods involving human subjects were approved by the ethics committee of the Bavarian Chamber of Physicians, Munich (EC No. 06068). LLS: In accordance with the Declaration of Helsinki, we obtained informed consent from all participants prior to their entering the study. Good clinical practice guidelines were maintained. The study protocol was approved by the ethical committee of the Leiden University Medical Center before the start of the study (P01.113).
