: Informed consent for autopsy and retention of tissues for research purposes was obtained in written format from all patients antemortem or, if not possible, from their spouses postmortem. No minors participated in this study. To ensure anonymization, at the time of utilization for this research project, all cases were assigned a unique identifier with the master list correlating the unique identifiers maintained in a locked cabinet in the office of the principal investigator (MJS) as per HSREB requirements.
