: Patients were informed that participation was voluntary and that survey respondents would remain anonymous, by written and oral explanation. They were also informed that they would not be disadvantaged in any way if they refused to participate. All the participants consented to the review of their medical records. Patients who signed the consent form and returned a completed questionnaire were considered to have agreed to participate in the survey. The study was reviewed and approved by the Ethical Committees of Saitama Prefectural University and Cancer Institute Hospital (No. 190045 and 2007-1072).
