: All procedures performed were in accordance with the ethical standards of the institutional and/or national research committee and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. The questioning was performed anonymously during the regular quality assessment of medical care in a large outpatient ophthalmologic tertiary centre. Anonymous data collection and analysis do not require ethical permission of the local ethical committee.
