: The study is based on registers with de-identified data. A part of the cohort has answered a questionnaire which contained information about the cohort study and how the information would be used in our register studies, they also received a separate letter with information about the cohort study and answering/sending the questionnaire to us has been regarded as informed consent. All approved by the Ethical Board.
