Ethics approval: According to Danish law (Act on Research Ethics Review of Health Research Projects, Sect. 14(2)), questionnaire- and register-based studies that do not involve collection of human biological material are not subject to approval by a research ethics committee. Therefore, no approval was required for this study ( ). All procedures performed in this study were in accordance with Danish ethical standards and with the Helsinki Declaration. Consent to participate in the study was informed, specific, voluntary, and explicit in accordance with guidelines from the Danish Data Protection Agency. The information to the participant was provided in a letter together with the link to the questionnaire. The participants were informed that by responding to the questionnaire, they gave active consent for their participation and use of their data in the study. They were furthermore informed that consent could be withdrawn at any time.
