: All procedures performed were in accordance with the ethical standards of the Health Research Ethics Committee of Stellenbosch University (reference S14/10/217), the Human Research Ethics Committee of the University of Cape Town (reference 184/2015), and with the 1964 Helsinki declaration and its later amendments. Informed consent was obtained from the parents or guardians of all individual participants included in the study’s prospective group. Assent was obtained from children of appropriate age and understanding. For the group of children recruited retrospectively, formal consent was not required.
