: All procedures performed in phase 1 of this study was approved by the University of Westminster’s Faculty Research Ethics Committee and has been performed in accordance with the ethical standards as laid down in the 1964 Declaration of Helsinki and its later amendments or comparable ethical standards. Informed consent was obtained from all individual participants included in phase 1. For phases 2 and 3 of this study, formal consent was not required.
