Ethical approval for this study was not required, as the Dutch Act on Medical Research involving Human Subjects does not apply to assessments that are part of routine clinical practice. All patients provided informed consent and had the opportunity to sign an op-out clause, so that in that case even anonymized data could not be used for other purposes such as scientific research. No one chose to opt out. For this study, fully anonymized and de-identified data were obtained for analyses and report.
