: This study was conducted as a multicenter, randomized comparative study in compliance with the ethical principles of the Declaration of Helsinki, according to the Ethical Guidelines for Clinical Research (Ministry of Health, Labour and Welfare Ministerial Notification no. 415), after approval of the ethics committees of the 16 participating facilities. The protocol for this research project was approved by a suitably constituted Ethics Committee of the University of Tokyo hospital, Approval no. P2013026-11X. Informed consent was obtained from the subject(s) and/or guardian(s). This study is registered at the Clinical Trials Registry in the University Hospital Medical Information Network (UMIN) (UMIN000014042).
