: All procedures performed in studies involving human participants were in accordance with the ethical standards of the National Danish Research Ethics Committee (Project ID: H-21047703) and the Swedish Ethical Review Authority (DNR 2017/1760–31 and 2025–03035-02), and with the 1964 Helsinki Declaration and its later amendments or comparable ethical standards.
: The Danish cohort was prospectively collected as part of routine clinical data acquisition and approved by the National Danish Research Ethics Committee. The Swedish cohort was based on retrospectively collected clinical data approved by the Swedish Ethical Review Authority. In both cohorts, all data were de-identified prior to analysis, and the requirement for individual patient consent to participate was waived in accordance with national regulations.
