: The Millennium Cohort Study (MCS) received ethical approval from the National Health Service Research Ethics Committee at each sweep of data collection. Ethical approval for the Avon Longitudinal Study of Parents and Children (ALSAPC) was obtained from the ALSPAC Law and Ethics Committee and Local Research Ethics Committees.
: Informed consent for MCS was provided by parents/caregivers, and children agreed to participate. From age 16, cohort members provided verbal informed consent to participate in the study.
