: The Danish National Birth Cohort (DNBC) was approved by the Danish Data Protection Agency and the Committee on Health Research Ethics under case no. (KF) 01-471/94. Data handling in the DNBC has been approved by Statens Serum Institut (SSI) under ref. no 18/04608 and is covered by the general approval (Fællesanmeldelse) given to SSI. The 18-year follow-up was approved under ref. no 2015-41-3961. The DNBC participants were enrolled by informed consent.
