: All procedures in this study were conducted according to the principles of the Declaration of Helsinki and were approved by the Medical Ethics Committee of Osaka University School of Medicine (No.08218. Date:13 January 2015). We obtained informed consent from the patients after Institutional Review Board (IRB) approval before the procedures. Because of the retrospective study, we waived the requirement in the cases before IRB approval for written informed consent from patients; however, we excluded patients who refused to participate in the study when contacted for follow-up.
