: The study was approved by the local ethics board of the hospital and informed consent was waived as this was a retrospective chart review. Data were collected form electronic medical charts using identification numbers. After data extraction, cases were encoded by serial numbers and saved anonymously. The list linking the study code with the participant’s details is stored in the investigator’s computer. There is no need for informed consent for participants.
