: This study was approved by the local ethics committee (98/13) and is registered in the German Clinical Trials Register (DRKS00004842).
: Patient consent was waived since the study does not report on primary research. All data analysed were collected as part of routine diagnosis and treatment. Our analysis looked retrospectively at outcomes for a large cohort of patients treated. This was done internally as part of an audit/evaluation, to improve our quality of care. All data used in the publication has been anonymised. The ethics commission of the Albert-Ludwigs-University Freiburg approved the study design and the omission of informed consent.
