: This study and its components were designed in accordance with the ethical standards of the Swedish National Research Committee (Swedish Research Council ) and the 1964 Helsinki Declaration and its later amendments. The Ethics Committee of Mid Sweden University approved the initial interviews for the questionnaire development (MIUN 2018/6) and the full study (MIUN 2019/6). All participants were informed of the aim of the study and the intent to publish its results, and were assured that confidentiality would be safeguarded.
