: According to Danish law, approval by written consent is not required for studies using questionnaire and register data; only trials are required to collect parental consent (Act No. 429 of 31 May 2000). Parents had the option to waive their children’s participation in the survey in 2004 and thus refrain from future participation in the survey. All data were analysed anonymously.
