: All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional research committee and with the 1964 Helsinki Declaration and its later amendments or comparable ethical standards. This study was approved by the Institutional Review Board at the University of Miami Miller School of Medicine. Prior to initiating the study, approval was obtained from the Institutional Review Board at the US site and participating Argentina sites. Prior to enrollment and provision of informed consent, physician participants were provided detailed information on study aims and assured of the confidentiality of study records. Signed informed consent was obtained by the study coordinators at each site prior to any study related assessments, and at the time of consent, participants were reminded of the risk of potential loss of confidentiality due to the low number of physicians at each site.
