: This study was approved by the Institutional Review Board at Washington University in St. Louis (IRB # 201704066) and the in-country local IRBs in Uganda: Makerere University School of Public Health Review Committee (Protocol # 210), and Uganda National Council of Science and Technology (UNCST, SS 2969).
