: This quality improvement activity was confirmed to be non-research by the Veterans Affairs National Director of Inpatient Mental Health Services and no ethics approval is required. The project was designed to inform the quality improvement efforts that are part of ongoing inpatient mental health policy reviews.
: All survey and interview respondents were informed of the voluntary and confidential nature as well as the purpose of the quality improvement project prior to their participation.
: The study design and reporting were guided by the SQUIRE checklist for quality improvement projects. However, no intervention was implemented in this project. The SRQR guidelines were followed for design and reporting of qualitative data. However, because this project is considered non-research, ethics approval is not required.
