: This research study was conducted retrospectively utilizing a de-identified database and required no patient contact. We consulted extensively with the IRB of Mayo Clinic, who determined our study to be exempt from the requirement of IRB approval. An IRB official waiver of ethical approval was granted from the IRB of Mayo Clinic. Patient consent to participate/publish was not required because of the institutional IRB waiver.
