: This is a minimal risk study, and a waiver of consent documentation was approved by the University of Arkansas for Medical Sciences Institutional Review Board. Consent information was provided to study participants on paper or electronically in Research Electronic Data Capture (REDCap) Suite via a study information sheet prior to taking the survey or conducting the focus group interviews. The study information sheet provided participants a detailed description of the study including key information such as: requirements for participation, the voluntary nature of participation, rights for refusal of participation, incentives for participation, risks of participation (minimal risk of loss of confidentiality), and the process to withdraw. Participants were given time to ask questions and were provided contact information for the Principal Investigator should they have questions later.

: The University of Arkansas for Medical Sciences (UAMS) Institutional Review Board (IRB) oversees research that involves human participants with the goal of protecting participant rights, safety, and welfare by ensuring compliance with laws and regulations that pertain to research as well as with the ethical standards described in IRB and institutional policies. The IRB includes four diverse committees of reviewers and a professional office staff, who collaboratively review and approve human subject research. The IRB oversees approximately 1450 open protocols and serves as the IRB of record for UAMS and Arkansas Childrens. The IRB also collaborates with multiple institutions and independent review boards across the country. The UAMS IRB has been fully accredited by the Association for the Accreditation of Human Research Protection Programs since 2005. All study materials and procedures were approved under expedited review by the UAMS IRB (IRB#273509).
