The University of Chicago Crown Family School of Social Work, Policy, and Practice and Chapin Hall Institutional Review Board approved both studies outlined in this study. The IRBs determined that the mystery shopper study outlined in this paper is exempt, given the minimal risk level to participants.

: Verbal informed consent was obtained from individuals who participated in qualitative interviews. A waiver of informed consent was obtained from the IRB for the Federally Qualified Health Centers and Community Mental Health Centers participating in the mystery shopper study.
