The Medical Ethics Committee of Amsterdam Academic Medical Center declared that the Medical Research Involving Human Subjects Act (WMO) did not apply to this study (reference number: 2020.474), therefore an official approval was not required. Subsequently, each organization received approval from their own local Scientific Review Board. All participants were informed about the content of this study and the potential risks before completing questionnaires. Informed and signed consent was obtained from all participating patients.
