: We confirm that we complied with the guidelines for human studies and that the research was conducted ethically following the World Medical Association Declaration of Helsinki of 1975, as revised in 2008. Yet, ethical approval was not obtained for the present retrospective study because the data were routinely collected as a standard clinic diagnostic procedure to ensure quality assurance and did not involve prospective evaluation. Nevertheless, all patients gave informed consent to anonymous evaluations of their routinely collected data.
