: Ethical approval is not required for registry-based studies in Denmark or Finland. However, the study was approved by the Danish Data Protection Agency and by the Institutional Review Board of the Finnish Institute for Health and Welfare. Study approval was obtained from the Regional Ethics Committee, South-East, in Norway and from the Regional Ethical Review Board, Stockholm, in Sweden.
