: We received permission to access the Danish National Patient Register from the Danish Health Data Authority (Danish: Sundhedsdatastyrelsen). Permission to use data from other registers was acquired from Statistics Denmark. According to the Danish research legislation, ethical committee evaluation is not required for register-based retrospective studies. The data are pseudonymized by Statistics Denmark and the authors do not have access to the pseudonymization key. The data are processed and stored in a safe remote-controlled data environment.
