: The study was registered with the Danish Data Protection Agency at Aarhus University (No 2016-051-000001-2587) and approved by Statistics Denmark and the Danish Health Data Authority. According to Danish law, informed consent or ethical approval is not required for register-based studies in Denmark. For survey participants, information about the survey was provided to potential participants in writing. All survey participants were informed that participation was voluntary and that their survey data would be linked to the registers for research purposes.
