: Approval for the primary study was granted by the Ethics Committee of the University of Cape Town. The current study  used an open access (publicly available) data set and does not require further ethical approval.

: Data were collected using telephone surveys. Therefore a verbal consent obtained from the participants by asking if they want to participate the survey. Those who said “yes” were included in the surveys.
