: Ethical approval for MCS was gained from NHS Multi-Centre Ethics Committees. Ethical approval for the teacher survey was given by the Northern and Yorkshire Multi-Centre Research Ethics Committee (MREC) of the NHS. Further approvals were sought and given for carrying out the survey in schools. For England, the survey was approved by the Star Chamber in the Department for Children, Schools and Families.
