: Ethical approval for this study was provided by human research ethics committees at Princess Margaret Hospital for Children, La Trobe University, Sydney Children’s Hospital Network, Mater Health Services, the University of Queensland, and the University of Western Australia. This approval was deemed sufficient by the Victoria University of Wellington Human Ethics Committee. Each child’s parent or primary supporter supplied written informed consent for both themselves and their child to participate in the study. If parents determined that the child was mature enough to understand the study requirements, children aged seven years and older were given the option to provide written or verbal assent.
