: G.H. and R.V. coordinated the implementation and evaluation of KiVa in the Netherlands. Program dissemination is done by a company (www.kivaschool.nl). C.S. led the development of the KiVa program and its implementation in Finland. The remaining authors declare that they have no conflicts of interest.

The Dutch law did not require IRB permission for this kind of research, and an Internal Review Board was not established at the Department of Sociology at the time of data collection (2012-2014).
