: The author is employed by the parent organization of the two study sites, and significant steps were taken to ensure there was no connection between treatment delivery and data collection, and evaluation methods.
: The study was conducted in compliance with all rules and regulations related to research with human subjects. Approval for the study and monitoring of the study was provided by the Institutional Review Board at the University of Detroit Mercy.
: Approved informed consent procedures approved by the University of Detroit Mercy Institutional Review Board. Consent was provided by all participants in the study, and in addition to consent, assent was provided by all participants under the age of 18.
