: The co-ordinating ethics committee of the Helsinki and Uusimaa Hospital District approved the implementation of the study. In addition, a research permit was obtained from the National Institute for Health and Welfare (THL) to utilize patient records and sickness absence statistics. The study followed the guidelines of the Research Ethics Advisory Board (ExternalRef removed).
