: The study fulfilled the ethical requirements and followed the guidelines of the Ethical Review Board in the Humanities and Social and Behavioural Sciences at the University of Helsinki. All procedures performed in studies involving human participants were in accordance with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. The Ethics Committee of the Finnish Institute for Health and Welfare approved the study design for each data collection.
: The agency for education in each municipality approved the study, and we obtained permission from the local school authorities. Adolescents were informed orally and in the first page of the questionnaire that the participation was voluntary. By filling the questionnaire, they gave their assent.
