: Ethical approval was provided for interviews with patients by the regional ethical review board of Gothenburg (dnr 247 − 15). Written informed consent was collected prior to interviews. The interviews with professional actors about their area of expertise did not pertain to sensitive data as defined by Article 9 of the EU’s General Data Protection Regulation (Regulation (EU) 2016/679 of the European Parliament and of the Council). Therefore, they do not fall under the Swedish Act concerning the ethical review of research involving humans (SCS :460) requiring ethical approval. Verbal informed consent was collected prior to interviews.
