: Written, informed consent from patients and next of kin was collected before inclusion in the study. De-identified data was immediately stored in a protected area for sensitive data (TSD) at the University of Oslo. The study was approved by the Regional Committee for Medical and Health Research Ethics (Ref.No 420920/REK south-eastern C), Sikt (Ref.No 919319), and by the data protection office at the university hospital. A small gift (value of 50 NOK – 4 EUR or 5 USD) was given to the patients after the interviews.
