: The Regional Committee for Medical and Health Research Ethics, Western Norway approved the study. The Norwegian Directorate for Children, Youth and Family Affairs provided exemptions from confidentiality for caseworkers and carers. In accordance with the Norwegian ethics requirement, oral consent is required from children aged 12 years or older.
