: The anonymous and voluntary online survey of the CoWELL project did not require ethical approval according to Swiss legislation. For the collection of HRQOL data in the Swiss general population, we received ethical approval from the Ethics Committee of Northwest and Central Switzerland (EKNZ 2015-075; 26 March 2015). Both surveys were conducted in line with the ethical principles of the WMA Declaration of Helsinki.
