: The study was conducted upon receipt of ethical approval from Lagos State University Teaching Hospital (LASUTH) Health Research Ethics Committee (Reference number: LREC/10/06/460). Permission to conduct the study was also obtained from the Lagos State Ministry of Health as well as from the management of the individual health facilities.
: A written informed consent was obtained from the respondents prior to the study. For respondents under the age of 18 years, an assent was obtained from them, while consent was obtained from their parents/guardians. The goals and objectives of the study were explained to study participants, and they were also informed that participation or non-participation in the study would have no impact on provision of healthcare. Information gotten was also anonymized by removing personal identifiers. Participation in the study was voluntary, and individuals were free to withdraw or stop the interview at any time.
