: The University of Pittsburgh (Pitt) institutional review board (IRB) approved this expedited study.
: The Pitt IRB approved the waiver for consent of patients to participate. Reasons included that this retrospective research project spanned over 5 years (2012–2017) of collecting the specified protected health information of the patients who met inclusion into the study. Majority of these patients, who are included under this waiver request, may no longer be treated by UPMC, relocated to somewhere outside of Pittsburgh, or have outdated contact information. Due to these reasons, obtaining a HIPAA authorization from these patients for research purposes was not feasible. However, with this waiver request, protected health information was de-identified in a separate document and then deleted by the research investigators upon completion of the project, leaving only de-identified data. Due to all data being de-identified in this study and that Pitt IRB approved waiver for consent to participate, consent for publication was also not feasible to obtain.
